  2. check must be made out to RK Management, Inc. and referenced to EPA Renovator Class


2. Registrations paid by Cash should:

  1. submit  this form, or fax to 214-341-6088 or email to dndmctran@gmail.com

  •   payment must be received one week prior to the start of class
  •   applicant will receive a seat in class after payment is received and emailed/mailed a confirmation   
      of payment
  •   applicant must bring a valid driver’s license or ID to class
